A 51-year-old woman visited our pain clinic with a history of continuous and paroxysmal pain in the right suprapubic area, lower back, and thigh for two years and six months. She was admitted to the hospital because of this pain and treated with nerve blocks, and showed some improvement for a short period of time. During her hospital stay, however, we noticed uncontrolled blood pressure and an associated hormonal imbalance. Computed tomography showed a paraganglioma in the retroperitoneal space. The patient¡¯s symptoms resolved upon surgical removal of the tumor. This case demonstrates the necessity of considering other approaches in the search for the cause of chronic pain in situations in which responses to treatment differ from the clinician¡¯s expectation. Appropriate care for patients with chronic pain must include rechecking past history, closed history taking, symptom monitoring, and proper evaluation.
